Establish Care Visits

(in-person only)

All establish care visits are in person. We feel this is the best way to develop a great provider/patient relationship. These appointments will provide a full complete history and physical exam. Your plan of care will be individualized just for you based upon your current needs and how we can better help you to achieve maximum health and joy to live an abundant life.

Follow-Up Visits (Chronic Care)

(offered in-person or virtually)

These appointments may be either in person or by telehealth. These appointments will allow you as a patient to update Kim on any new concerns you have as well as review your past chronic care issues and current plan of care.

Acute Care Visits

(offered in-person or virtually)

These appointments are to address any acute care needs you may have. We know the importance of being able to see your provider quickly when needed. We make it a goal to see each patient either the same day or next day depending upon the time of the call.

Wellness Exams/Sports and Work Physicals

(in-person only)

Wellness exams will be offered once a year. We are firm believers in preventing disease before it happens. These appointments will focus on prevention as well as what labs or testing need to be completed to ensure maximum health for your future. Sports and work physicals will be completed along with any forms on an as needed basis. (please note we do not do DOT physicals at this time)

Specialist Provider-Provider consultations

(offered in-person and virtually)

We know the stress of trying to find a specialist that can help manage your in-depth chronic care issues. That is why we have teamed with a great group of specialists who offer provider to provider consultations for greatly reduced cost on your care when needed. You will first meet with primary care provider Kim Kemnitzer, FNP to review which consultation would be best for your care. Kim then will meet and consult with the provider of your choice so that you can continue to get excellent care right here at home.

Nutrition and Weight loss management

(offered in-person and virtually)

These appointments will be approximately 30 minutes and will be providing education on nutrition, diets, as well as creating a plan for you individually to reach maximum health. Medications for weight loss will be discussed on an individual basis.

Diabetic Education and Consultation

(offered in-person and virtually)

These consultations will be for Diabetic Education. These appointments will be approximately 30 minutes each and will review what diabetes is, medications used to manage diabetes, preventative care, labs specific for diabetes, as well as nutrition, diet, exercise, and lifestyle changes that will help you live a full and happy life.

Mental Health and Mindfulness Consultation

(offered in-person and virtually)

These consultations will focus solely on mental health and will be for approximately 30 minutes. We understand the burden that anxiety, depression, PTSD, stress, bipolar disorder and so much more can have on your life. These appointments will focus on ways that you can more fully live a full life through lifestyle modifications, mindfulness, mediation, exercise, nutrition, as well as medications if needed. (please note that controlled substances will not be routinely used to control symptoms)

Labs

We will be offering significantly reduced cash-pay prices on labs for all patients. Members will be able to receive labs at cost with no lab draw fees. As a patient you are free to have labs drawn at an outside facility and utilize insurance.
